What We Offer

Our services are built around the needs, preferences, and goals of each individual we support. Whether it's help at home, in the community, or at our day program, we focus on building independence and improving quality of life. Click any service below to learn more.

Residential Habilitation Supports (RHS)

24/7 staff support in a structured home setting. We promote life skills, community access, and a sense of belonging in a safe and supportive environment.

Supported Living  (SLN/SLH)

One-on-one support provided in the client’s own home or apartment. Services are flexible and based on the client’s goals and daily needs.

Day Program Services (DSG, DSP)

Structured daytime activities designed to build social, daily living, and community participation skills. Transportation may be provided.

Transportation Services (MTP)

Reliable, safe transportation to and from day programs, appointments, and community outings — always staffed by trained, qualified drivers.

Personal Budget Assistance (PBA)

Guidance in managing personal finances and ensuring bills and basic needs are taken care of. Offered for clients who need help managing Social Security funds.

Employment Preparation Services (EPR)

Skill-building for those looking to pursue job readiness and workplace independence. Includes resume support, coaching, and community-based assessments.

Person-Centered in Every Setting
  • All services are delivered based on the individual’s Person-Centered Support Plan (PCSP)
